· The absent parent’s social security

· Marriage license
· Certified
divorce/separation documents and the child support orders
· Birth Certificate of the children
· Last 3 paycheck stubs
· If you were not married to the father
of your child(ren), please bring the following if available:
 Photographs of the father
with the child(ren)
Letters or cards from the
father mentioning or making reference to the child(ren)
Names and address of
persons who have knowledge of the alleged father's relationship with
the child(ren)
Names and address of
anyone who has knowledge of your relationship with the father.
